Reassessment
What it means in California's IHSS program, in plain English.

The periodic in-home evaluation California counties conduct to redetermine a recipient's authorized IHSS hours — required at least once per year, and any time a recipient requests one due to a change in their needs. A reassessment follows the same Functional Index and Hourly Task Guidelines process as the original assessment, and the results are documented on an updated SOC 293.
