IHSS Errands and Appointments

This is one of the most common situations people describe without realizing it already fits IHSS: an older adult or someone with a health condition who's still managing day-to-day, but who genuinely needs a family member or friend to handle grocery shopping and get them to and from doctor appointments — a few hours a week, nothing dramatic, just consistent help that's become part of the routine.

What This Falls Under

Grocery shopping and errands are covered under Related Services — the same category that includes meal preparation and cleanup. Getting to and from medical appointments, plus staying with the recipient during the visit if needed, falls under Accompaniment. Both are standard, well-established IHSS service categories, not edge cases.

"Does Informal Help Count as IHSS?"

Not automatically — and this is the part that trips people up. Help from a family member only becomes paid IHSS help once the recipient is formally assessed and approved by their county, and the family member is enrolled as their provider. The help itself doesn't change; what changes is that it gets recognized and paid through the program instead of being unpaid.

Family members can be paid IHSS providers — including spouses in some cases, and parents of adult children. It's one of the most common misconceptions about the program: people assume a "real" care provider has to be an outside hire, when in practice the person already helping is often exactly who ends up enrolled.

What This Doesn't Tell You

How many hours a week of shopping and appointment help translates to in authorized IHSS time depends on the recipient's overall assessment, not just this one task in isolation. This scenario tells you which categories apply — not what the final hours will be. That's determined at the county assessment.
