A common pattern: someone is largely managing on their own, but balance or strength has declined to the point where bathing alone is a real fall risk — stepping over the tub edge, standing on wet tile, or simply not having the strength to get back up safely.

What This Falls Under

This is Personal Care — Bathing, one of the core IHSS task categories. How much help is authorized depends on the actual level of assistance needed, assessed on a 1-5 scale: from verbal reminders and standby supervision at the lower end, up to full hands-on physical assistance at the higher end. The same category covers everything from someone who just needs another person present in case something goes wrong, to someone who needs to be physically bathed.

Bathing rarely stands alone in an assessment. If mobility is part of what's changed, Ambulation (moving safely from room to room) and Transfer (getting in and out of the tub, on and off the toilet) are usually evaluated at the same visit, since they tend to be connected — someone who needs standby help in the shower often needs it getting there too.

If the concern is specifically about falling, mention that directly during the assessment — "I'm worried about them falling in the shower" gives the social worker something concrete to evaluate, rather than a general "they need help bathing."
